The role requires high level leadership and communication skills, to undertake a review of model of care. The need for negotiation and delegations skills with existing District programs and to motivate and coordinate a team of part-time staff working across the Central Coast. Development of revised policy and procedure that ensures patient and carer partnership in care planning and delivery as part of as traditional medical model of care.
Oversee and drive the development and implementation of new ways of working within the HITH program that align within the constraints of the funding agreement and reporting functions from Family and Community Services (FACs) Carers Investment Project.

What you’ll be doing
Lead the Partnering with Carers in Hospital In The Home (HITH) project across CCLHD, developing change and adoption strategies that identify caring relationships, assess carer capacity and develop tools and resources to address the practical and psychological needs of carers participating in the HITH program.

The role will manage the three year project, which will be responsible for identifying and developing innovative changes to business processes, systems and technology, job roles and LHD structures to ensure the successful engagement of carers as partners across the HITH program.

The role will provide formal reporting on project milestones and targeted expenditure to the Project Sponsor and External funding organisation as per the Project Contractual Agreement.
