What you’ll be doing
The Sustaining NSW Families Program (SNF) Child and Family Health Nurse (CFHN), practices at an advanced level of nursing, implementing a sustained health home visiting program which commences in pregnancy or up to four weeks after the birth of the baby and completes at the child’s second birthday. The nurse works in a primary healthcare model with families who have been identified through the Safestart process.

The SNF Nurse will perform health and development surveillance and screening of infants/children in accordance with the Personal Health Record (PHR) schedule of developmental checks. The SNF Nurse also completes assessments and measures as per the SNF program home visiting schedule ( Sustaining NSW Program Handbook Guidelines) using screening tools including comprehensive health assessment, SAFESTART psychosocial assessment, NCAST, HOME Inventory, ASQ and ASQ-SE to inform practice. The nurse works as part of a multidisciplinary team, and participates in regular multidisciplinary case reviews to plan care.

The SNF Nurse promotes, encourages and supports breastfeeding and the maintenance of lactation, uses a family partnership and anticipatory guidance approach to promote optimal infant and maternal mental health, child development and positive parent child relationships.
