The Opportunity

We are a looking for encouraging people to provide support to our Indigenous customers on their recovery journeys in community outreach and centre based settings.

The role focuses on empowering our customers to be independent community members who are connected to their family, friends and culture. By promoting positive lifestyle choices, we aim to enhance the wellbeing of our customers.

This role is for someone who wants to help improve the cultural and family connections of people in the indigenous community. It is for someone who is willing to learn and grow in a role that is focused on empowering people.

The position is a max term full time opportunity based in the Hunter Newcastle region.

You will report to the rostered site Team Leader. Key responsibilities will include:

  • Planning with customers their support recovery journey
  • Connecting customers with their families and culture
  • Person centred case management
  • Developing individual support plans
  • Encouraging independent community participation
  • Building positive networks in the community
  • Completing detailed administration, notes and reports on customer progress

Skill and Experience:

  • Strong communication and interpersonal skills
  • Passion for the sector and supporting vulnerable people
  • Technical literacy with proven ability to use Microsoft Office and systems for reporting
  • Minimum Cert IV in Mental Health or Disability or equivalent industry experience
  • A current NSW Driver Licence
  • Willingness to obtain an Australian Federal Police Check, WWCC and first aid certificate that meet New Horizons employment criteria prior to commencing with us.
